New Student Enrollment Process

1) Complete the following forms:
a) Application for Enrollment (Download form here.)
b) Financial Agreement (Download form here.)
c) Medical Examination (Download form here.)
d) Consent for Treatment (Download form here.)
e) Character References (Submit 2) (Download form here.)
f) Approved Transportation (Download form here.)
2) Obtain transcript or report card from previous school.
(A Request for Records form is available here.)
3) Make a copy of the following documents:
a) Birth certificate
b) Social Security card
4) Submit above to school with payment of registration fee and 1st month's tuition at time of registration.
