Consent for Treatment

Approved Transportation
|

|
Obtain transcript or report card from previous school.

|
Make a copy of the following documents:

Birth certificate

Social Security card
|

4)
|
Submit above to school with payment of registration fee and 1st month's tuition at time of registration.
