New Student Enrollment Process

1) Complete the following forms:
a) Application for Admission (Download form here.)
b) Financial Agreement (Download form here.)
c) Medical Examination (Download form here.)
d) Consent to Treatment (Download form here.)
e) Character Reference (Submit 2) (Download form here.)
f) Emergency Contacts (Download form here.)
g) Internet Access & Use (Download form here.)
2) Obtain transcript or report card from previous school.
(A Request for Records form is available here.)
3) Make a copy of the following documents:
a) Birth certificate
b) Social Security card
4) Submit above to school with payment of registration fee and tuition at time of registration.
5) Get school supplies list (download here.)
